Slabaugh, MD, Nicole A. Friel, BS and Brian J. Cole, MD, MBA published in The Journal of Bone and Joint Surgery (American). 2010;92:186-189. Below is an excerpt: "Chondrolysis is a devastating complication of surgery on any joint, resulting in the rapid and extensive destruction of the articular cartilage. Associated with this destruction is an equally large inflammatory response that causes adhesions in the lining of the involved joint. The inflammatory response manifests as arthrofibrosis and is typically seen within six months after the operation. Management of this unique complication is difficult because most patients are very young and have active lifestyles. Chondrolysis causes more immediate symptoms of pain and limited range of motion than does osteoarthritis, which often takes years to become symptomatic. The exact cause of postoperative chondrolysis is unknown; however, several hypotheses have been entertained, such as the use of thermal treatment1, occult infection with Propionibacterium acnes2,3, idiopathic or iatrogenic osteochondral injury, high temperature of fluid during arthroscopic irrigation4, improperly placed implants, and the use of nonsteroidal anti-inflammatory medications." Another interesting study is called, "Postarthroscopic Chondrolysis of the Glenohumeral Joint" by Jesus Rey II, MD, John R. Reineck, MD, Sumant G. Krishnan, MD, and Wayne Z. Burkhead Jr, MD – published in the Journal of Hand Therapy - Volume 20, Issue 2, Pages 98-103 (June 2009). Here is a excerpt: "Chondrolysis is a process of unknown etiology characterized by rapid progressive cartilage destruction with resultant pain and loss of motion. Chondrolysis after arthroscopic surgery of the shoulder is multifactorial. Recent reports suggest that the heat from radiofrequency ablation devices, certain bioabsorbable implants, and the local anesthetics delivered in excessive doses via pain pumps could play a role in its development. We review the current understanding of this condition and our institution's experience with glenohumeral chondrolysis. Our discussion will serve almost exclusively as a caveat to surgeons of a potentially catastrophic complication in the young patient."
