Could any American please explain the "grid" system of mapping US cities, & sizes of city blocks, etc.?

Is this a standard system applied throughout the USA? What size is a "lot", or a "block"? How does the grid system work where nature doesn't co-operate (rivers, hillsides, etc)?

There isn't really a "standard system" and accomdations for things like rivers & hillsides were simply made at the time. Many American cities had to re-number their street grids in the late 19th or early 20th centuries to account for growth.
